New Patient Registration

New Patients are Always Welcome to Join Clee Medical Centre

If you or anyone you know is thinking of changing doctors, or have recently moved into the area, please speak to one of our receptionists who will explain the quick and easy registration process to you.

Alternatively all the forms are available to download here.

Please print off and complete a GMS1 form for each person and also a new patient questionnaire.

Please complete and sign both forms and bring them to the surgery along with some photo ID and proof of address, such as a utility bill and full list of current medications.  If you wish to email please ensure these items are sent with the  forms to nel.b81015@nhs.net.

We are unable to accept patients outside of the North East Lincolnshire Boundary.

INCOMPLETE REGISTRATION  REQUESTS WILL NOT BE ACCEPTED.

Update your Address Details

Have you recently moved address?

Have you changed your mobile phone number?

It is important that we have your correct personal information in case we need to contact you.
