Online Consultations

Please use the link opposite for non-urgent requests only. Response will be made within 48 working hours (except weekends and bank holidays).

The requests will be check for arrival between 8 am and 1.30 pm Monday to Friday.  Requests sent after 1.30pm will be counted from the next day 8am.  

Please ensure the form is completed accurately and follow guidance should you be re directed to seek alternative help.

Community Diagnostic Centre - Blood Test Appointments

For patients who have been requested to obtain blood sampling by the hospital, or those requiring specific times, there is a new Community Diagnostic Centre currently housed in Freshney Green Medical centre where you may book for your blood test via the link.

Please ensure you take a blood request form with you to the appointment or the procedure can not be completed.

 

 

Urgent Appointment

To request an urgent appointment for today or tomorrow (Monday to Friday):

  • phone us on 01472 697257, Monday to Friday from 8am

When you get in touch, we’ll ask what you need help with.

We will use the information you give us to choose the most suitable doctor, nurse or health professional to help you.

Routine Appointment

To request a routine appointment in the next 7 days:

  • use our online booking system - SystmOnline
  • Use our accurx self book links when invited by the practice.
  • use your NHS account (through the NHS website or NHS App)
  • phone us on 01472 697257, Monday to Friday from 8am
  • visit the surgery and speak with a receptionist, Monday to Friday from 9am to 5pm

When you get in touch, we’ll ask what you need help with.

We will use the information you give us to choose the most suitable doctor, nurse or health professional to help you.

Practice nurses

Please make an appointment with a Practice Nurse for:

  • Smears
  • Dressings
  • Injections including holiday vaccinations, B12 and HPV
  • Blood pressure checks
  • Swabs
  • Repeat contraception, both oral and injection
  • Health promotion
  • New Patient checks
  • Patient health checks
  • Emergency contraception

We also have specially qualified nurses who deal with and can advise on:

  • Patients wishing to start or change their method of contraception
  • Lipid / cholesterol and diet advise
  • Leg ulcers / wound management
  • Baby vaccinations
  • Diabetes
  • COPD and Asthma
  • Chronic heart disease and strokes

Your appointment

However you choose to contact us, we may offer you a consultation:

  • by phone
  • face to face at the surgery
  • on a video call
  • by text or email

Appointments by phone, video call or by text or email can be more flexible and often means you get help sooner.

Cancelling an appointment

To cancel your appointment:

  • use your NHS account (through the NHS website or NHS App)
  • using the GP online systemSystmOnline
  • phone us on 01472 697257, Monday to Friday from 8am
  • reply CANCEL to your appointment reminder text message

Please note; persistent missed appointments without notice may result in you being removed from our practice register.

If you need help when we are closed

If you need medical help now, use NHS 111 online or Call 111.

NHS 111 online is for people aged 5 and over. Call 111 if you need help for a child under 5.

Call 999 in a medical or mental health emergency. This is when someone is seriously ill or injured and their life is at risk.

If you need help with your appointment

Please tell us:

  • if there’s a specific doctor, nurse or other health professional you would prefer to respond
  • if you would prefer to consult with the doctor or nurse by phone, face-to-face, by video call or by text or email
  • if you need an interpreter
  • if you have any other access or communication needs

Home visits

 

Patients whose medical condition prevents them getting to the surgery can be visited at home. Requests for home visits should be made by telephoning 01472 697257 before 10 am wherever possible.
Please remember that several patients can be seen at the surgery in the time it takes to do a home visit.

You can be visited at home by a community nurse if you are referred by your GP.

You should also be visited at home by a health visitor if you have recently had a baby.

Extended access

Evening and weekend appointments now available

From 1st October 2018 everyone in England will now benefit from access to general practice appointments in the evenings and weekends at a time that is most convenient to them. This is part of a national drive to help improve access to general practice and get the best possible outcomes for patients. Patients who are registered at this practice can now book an appointment to see a range of clinicians including GPs and nurses on weekday evenings (after 6.30pm) or at the weekend (on Saturday 9 to 5pm). All appointments will take place at Clee Medical Centre.

A Number of different surgeries in North East Lincolnshire participate in these extended access appointments so you may not necessarily see a GP or Nurse from Clee Medical Centre.

You can book routine review appointments for the following:

COPD / Asthma / Diabetic / Cervical Screening / Contraception renewals and many more depending on the clinician working the Clinic that week. 

You can also book our extended access clinics for routine Health Care appointments these include Blood tests / Blood Pressure checks / B12 Injections and so on. 

These appointments are booked in advanced and access to the surgery will only be granted if you have an appointment booked via Intercom.

Teaching Practice

Clee Medical Centre participates in the training of Medical Students from the Hull & York Medical School.

This means that at any one time we may have up to eight students at Medical School in the Practice.

We also train recently qualified Doctors during their General Practitioner attachment.

As part of this you may be asked to take part in a video consultation. If so, written permission will be required and it is totally your decision whether to allow filming or not. Only people directly involved in the assessment will see the video. You do not have to agree to your consultation being recorded, this is not a problem and will not affect you consultation in any way. Patients who do consent to having their consultation recorded can be assured that their confidentiality will be guaranteed, and the recording will be stored in a secure area.

We also train both pre and post registration Nurses across all the disciplines - Practice Nursing, District Nursing, Health Visiting & Nurse Practitioners as well as other health professionals. All patient contact with our students will be closely monitored.
